Before You Arrive: Please ensure you have filled all all intake forms, including past medical history, and consents prior to your appointment. Upon booking, you should receive an email confirmation as well as a link to all the forms. If you do not receive the email, please reach out to us and we will be happy to resend it.

Keep in Mind: Make sure you have had something to eat prior to your appointment. Please wear loose, comfortable clothing where sleeves and trouser legs can be comfortably rolled up. For cosmetic and facial appointments, please wear minimal make-up and a low-cut top to allow access to the neck area.
